From surface perceptions to deep organizational values — and how to tell the difference

Safety Culture vs. Safety Climate

These two terms get used interchangeably in practice, but they refer to genuinely different things — and conflating them leads to misdiagnosis.

Safety culture refers to the deep, enduring assembly of shared values, beliefs, attitudes, norms, and practices within an organization that shape how safety is perceived, prioritized, and enacted over time. It is relatively stable: it develops through repeated patterns of behavior, leadership decisions, and accumulated organizational experience. It is hard to observe directly.

Safety climate refers to employees' perceptions of the organizational safety environment at a specific point in time — how workers currently experience safety policies, procedures, and practices. It is the measurable surface expression of culture. You can survey for it.

The distinction matters in practice. An organization can show strong safety climate scores (workers currently perceive management takes safety seriously) while sitting on a weak underlying culture (there is no sustained commitment, just a recent audit). The reverse is also possible: a solid culture temporarily depressed during organizational stress or restructuring might produce poor climate scores that don't reflect the organization's actual underlying commitment.

When to use which

Use safety climate when you want a current-state diagnostic — a periodic check on how workers are experiencing safety at this moment. Use safety culture when you want to understand why those perceptions exist, whether change will stick, and what the organization's long-run trajectory looks like.

The IAEA's definition captures the depth of culture well: a strong safety culture is "that assembly of characteristics, attitudes and behaviours in individuals, organizations and institutions which establishes that, as an overriding priority, protection and safety issues receive the attention warranted by their significance." The phrase "overriding priority" is doing serious work here — it means safety cannot be traded off against schedule pressure or commercial interest when the stakes demand it.

Safety Culture Is an Organizational-Wide Phenomenon

It is tempting to think of safety culture as something that belongs to operations teams. It does not. Analysis of the Chernobyl disaster showed that safety culture deficiencies extended not just to plant operators, but across design bureaus, ministry-level administration, and regulatory bodies. The IAEA explicitly extended the concept to cover all types of activities at all stages in the lifetime of a technical system — including designers, engineers, constructors, equipment manufacturers, and regulatory bodies.

For engineering organizations, this means safety culture failures can originate in product decisions, architectural choices, or planning and estimation practices — long before any operational incident.

The Hudson Maturity Model

Patrick Hudson and Dianne Parker developed the most widely used diagnostic framework for safety culture: a five-level maturity model often called the "safety culture ladder." Developed from in-depth interviews with senior executives in high-risk industries, the model characterizes how organizations think about and manage safety at each stage.

The five levels are:

  1. Pathological — Safety exists only as a compliance exercise. The driving motivation is not getting caught, not preventing harm.
  2. Reactive — The organization responds to accidents, but only after they happen. Safety effort is driven by events, not by anticipation.
  3. Calculative — Systems and procedures are in place. Data is collected. But this is mostly procedural: the organization manages safety by counting and auditing rather than by understanding.
  4. Proactive — The organization actively seeks out problems before they cause harm. There is genuine curiosity about what might go wrong.
  5. Generative — Safety is fully integrated into how work is conceived and conducted. It is not a separate function; it is embedded in organizational identity.
The calculative trap

Many organizations plateau at level 3. They have audit programs, incident dashboards, and compliance checklists — and mistake this infrastructure for a safety culture. Research shows that incident-free records and procedural compliance can mask growing systemic vulnerabilities. Incidents and fatalities can follow years of clean metrics in aviation and construction precisely because the metrics were not capturing real safety capability.

The Four Cornerstones of Safety Culture

A safety culture that functions as more than a slogan rests on four interdependent cornerstones. These are not sequential stages to implement one at a time. Remove any one and the others degrade.

Informed culture is the organization's collective knowledge about hazards, how they are managed, and what factors affect operational safety. Workers and managers at all levels need adequate knowledge of potential dangers and the systems designed to address them — both formal knowledge (documented procedures, training, hazard registers) and tacit knowledge embedded in experienced people. Without an informed culture, the organization cannot exercise competent judgment about safety matters.

Just culture defines how the organization responds when things go wrong. Sidney Dekker's restorative just culture reframes accountability: instead of identifying who to punish, accountability means seeking an account of what happened — something to learn from, not something to impose consequences for. A just culture is not a blame-free culture. Reckless or deliberately unjustifiable unsafe acts still carry accountability. But unintentional errors are not punished, and individual responsibility is assessed in the context of contributing system factors.

Reporting culture is the atmosphere in which people feel confident reporting safety concerns, errors, and near-misses. Psychological safety is its foundation: the interpersonal conditions under which people can speak up about problems without fear of embarrassment or retribution. Reporting culture also depends on confidentiality protections and on workers believing that submitted information will actually be acted upon.

Learning culture is the organization's ability to convert the information produced by reporting into action. This requires not just collecting data, but analyzing issues, identifying root causes, implementing improvements, and sharing lessons across the organization — ensuring gains are not siloed to individual teams or locations.

Organizational learning in resilience engineering extends this further: learning is collective, multilevel, and multidimensional, encompassing not only incident investigation but learning from normal operations and successful adaptations.

The Enabling Role of Management Commitment

Management commitment is the single most critical factor in developing and sustaining safety culture. The influence chain is direct: top management commitment shapes supervisor commitment and safety training, which in turn shapes employee commitment and safety behavior. No safety system can compensate for the absence of visible, genuine leadership commitment.

Visibility matters. Resource allocation decisions, how scheduling conflicts are resolved when safety is involved, whether leaders personally participate in safety activities — these signals communicate organizational values far more powerfully than any posted policy. Research confirms that management commitment functions as an antecedent: it creates the conditions under which all four cornerstones can develop.

Just Culture vs. Blame-Free Culture

These are often confused, and the confusion is damaging. A blame-free culture treats all adverse outcomes as system failures and removes individual accountability entirely. A just culture is different: it distinguishes between human error (which deserves support and system redesign), at-risk behavior (which deserves coaching and context-addressing), and reckless behavior (which may warrant disciplinary action). The fairness of the distinction is what creates trust — and trust is what makes reporting possible.

Eliminating blame entirely without maintaining this distinction tends to obscure accountability for reckless behavior and can actually erode trust in the system, since workers see that gross negligence and honest mistakes are treated identically.

Annotated Case Study

The Normalization of Compliance at Three Mile Island

The Three Mile Island accident (1979) illustrates what happens when calculative safety culture meets unexpected conditions. The plant had extensive procedural systems, documented training requirements, and regulatory compliance records. On the surface, it was operating within all defined parameters.

When the accident sequence began, operators followed procedures designed for conditions that did not match what was actually occurring. The procedures were the product of a calculative culture: hazards had been enumerated and procedures written for those hazards. The system had not developed the informed culture necessary for operators to recognize when the situation had moved outside the boundaries those procedures were designed for. There was no organizational expectation that operators should reason independently about novel conditions.

What the cornerstones reveal: The plant had adequate procedural infrastructure (some informed culture) but lacked the learning culture and just culture necessary for the workforce to feel safe raising concerns or departing from procedure when evidence warranted it. Safety culture analysis of TMI and similar events consistently points to the same pattern: compliance systems that do not support reporting and learning leave systemic gaps invisible until they become accidents.

The management signal: Years of incident-free operation had produced confidence — in management, in regulators, and in operators — that the system was working. This is precisely the dynamic that compliance-focused safety culture is prone to: a clean record reads as evidence of safety, when it may instead reflect suppression of weak signals.

Common Misconceptions

"A good safety record means we have a strong safety culture." This is one of the most dangerous inferences in safety management. Research shows that incidents and fatalities can follow years of incident-free performance. An incident-free record may reflect genuine safety capability, or it may reflect incident suppression, metric gaming, or the absence of conditions serious enough to reveal existing vulnerabilities. A strong safety culture produces the capability to prevent harm; it does not simply produce low incident counts as a primary output.

"Safety culture belongs to the safety team." Culture spans every level and function — designers, engineers, managers, regulators. For software and platform engineering organizations, decisions made in product planning, capacity allocation, and architectural design carry cultural weight. Treating culture as a concern for a dedicated safety or reliability function is itself a sign of a calculative rather than generative culture.

"Just culture means no one gets held accountable." Just culture is explicitly not blame-free. The distinction is between appropriate and inappropriate accountability. Dekker's restorative framework maintains accountability — it redefines what accountability means: seeking understanding rather than seeking targets. Reckless behavior that disregards obvious risks remains within the scope of disciplinary response.

"Safety climate surveys measure safety culture." Safety climate is a snapshot of current perceptions; safety culture is the enduring set of values beneath those perceptions. A climate survey can be a useful input into cultural assessment, but it cannot substitute for it. An organization can score well on a climate survey after a high-profile leadership intervention while the underlying culture remains unchanged.

"The four cornerstones are a sequential implementation roadmap." The cornerstones are interdependent, not sequential. You cannot build a learning culture and then add reporting culture later. Without just culture, people do not report. Without reporting, there is no data to learn from. Without learning, informed culture cannot grow. Treating them as independent workstreams misses how each depends on the others being in place.

This exercise is designed to be done individually first, then discussed with a colleague or team.

Part 1 — Individual Reflection (15 minutes)

Consider a recent incident, near-miss, or reliability event in your organization. For each question, write a brief honest answer — not the official version, but what you actually observed:

  1. When the incident was reported, what was the first organizational response? Was it focused on understanding what happened, or on identifying who was responsible?
  2. Did people who reported concerns or errors receive support, or did they experience adverse consequences (formal or informal)?
  3. After the incident was investigated, what changed? Were improvements implemented and communicated broadly, or did the report get filed and forgotten?
  4. Do people in your organization proactively raise concerns before incidents happen, or do issues tend to surface only after something goes wrong?
  5. How visible is your management team's engagement with safety-related concerns? Does it affect how decisions get made under pressure?

Part 2 — Hudson Placement (5 minutes)

Based on your answers, place your organization on the Hudson ladder (pathological → reactive → calculative → proactive → generative). What is the evidence that supports your assessment? What evidence contradicts it?

Part 3 — Cornerstone Audit (10 minutes)

Rate the four cornerstones in your organization: informed, just, reporting, learning. Which is strongest? Which is the binding constraint — the one whose weakness is most limiting the others?
